This position will serve as a much-needed community resource in the City of Chandler and the Town of Gilbert whereby the incumbent: identifies, engages, supports and advocates for unsheltered individuals/families experiencing homelessness. This is accomplished through direct and indirect outreach in the community supported by referrals from various entities. This employee will support unsheltered persons to navigate community resources and facilitate access to
housing, services and resources to assist in ending homelessness. Additionally, the incumbent will provide case management their clients in a variety of instances including housing opportunities.
● Provide outreach and engagement services to individuals and families living without shelter in the community in traditional and non-traditional service settings such as sidewalks, parks and other community areas
● Operate one’s own transportation safely throughout the community
● Provide Case Management for individuals/families to navigate services including but not limited to: basic needs, transportation, securing benefits, mediation with families and property managers, paperwork, connection to mental health and recovery services, etc.
● Complete a Vulnerability Index-Service Prioritization and Decision Assistance (VI-SPDAT) checklist during community outreach and referral processes
● Identify and connect clients with emergency and bridge housing options; support with longer term housing search, selection and stability
● Assist clients to obtain documentation necessary to be "housing ready" (e.g. birth certificates, personal identification, social security card, income verification, etc.)
● Assist with the acquisition of furniture and move-in essentials, as needed
● Support program participants involved in the criminal justice system to navigate the process while advocating for services and sentences appropriate based on their circumstances and behavioral health background
● Maintain accurate and non-duplicated client files and enter data into the Homeless Management Information System (HMIS) according to the timeliness standards set by the Maricopa Regional Continuum of Care
● Assists with the annual Point-in-Time (PIT) Count of persons experiencing homelessness and other special events
● Participate in regional and local workgroups related to homelessness
● Attend weekly housing case conferencing provided through the Keys to Change Campus
● Work in a safe manner and escalate unsafe activity and conditions to the appropriate person/party
● Provide on-call support in a timely fashion and in accordance with the On-Call Policy.
● Follow oral and written instructions, asking questions and seeking guidance when needed
● Establish rapport with clients and build meaningful partnerships in the community
● Prepare clear and concise reports; maintain findings, documents, correspondence and related records
● Perform all work duties and activities in accordance with AZCEND policies and procedures
● Exercise confidentiality, sensitivity and professionalism while upholding organization values and goals
● Participate in on-going training and professional development
● Work collaboratively as a team member
● Other duties as assigned

Qualifications

● Knowledge of: chronically homeless populations; Maricopa Regional Coordinated Entry System and the Homeless Management Information System (HMIS); Emergency Shelter and Bridge Housing Resources; Regional Behavioral Health System, detox and recovery services; case management record keeping practices, techniques and procedures; public benefits and community resources; and, principles and practices of Harm Reduction and motivational interviewing, strongly preferred
● Efficient and professional communication, both orally and in writing
● Ability to respond to potentially stressful situations while using sound judgment
● Strong computer skills needed to interface internally and externally as well as utilize HMIS database, reports and other work completed with technology
● High School diploma or GED equivalency
● Two (2) year's experience working with people experiencing homelessness
● Valid Arizona Drivers License with acceptable driving record
● Any equivalent combination of experience and training which provides the knowledge and abilities necessary to perform the responsibilities of the position
● Ability to encourage teamwork and work cooperatively with others with multicultural sensitivity
● Work well independently with minimal supervision
● Bilingual English/Spanish preferred but not required
● Must be able to obtain a Level One fingerprint clearance card from DPS and pass a Central Registry background
check
