Job Contributions
- Process client applications for financial assistance from multiple programs and services
- Provide exceptional customer service with navigation to additional resources and create referrals for services in the department’s CMS
- Review program applications for eligibility with the requested program/service
- Maintains all required documents in the program’s electronic management information system
- Engages the customer in Evidence-Based coaching interviews and utilizes related tools to meet the client where they are and identify the most appropriate level of services
- Review program applications and documentation to determine customer’s eligibility with requested services
- Provide information on community resources, establish information and referral mechanisms, and provide program enrollment assistance to interested clients, including utility discounts and food/nutrition programs
- Attends community outreach and education events
- May require travel to various locations throughout Maricopa County
- Other duties as assigned
Working Conditions
- Must possess or have the ability to obtain a valid Arizona driver's license upon hire
- Arizona Department of Public Safety Level 1 Fingerprint Clearance Card, upon hire
- Work is routinely performed in an office environment, which may require long periods of sitting, reading, and typing
- Requires travel to program sites throughout Maricopa County
- The position requires travel to and from various county, client, and contractor locations during a scheduled workday, subject to County policies regarding the use of County vehicles and/or private vehicles used on County business
Qualifications
We Require
- Two years of case management experience
- Certificate, credential, or Associate degree in social work or related field
- An equivalent combination of post-secondary education and job-related experience may substitute for the required education year after year
